Full-Time Case Manager Opportunity

We are currently hiring for a Case Manager who will work directly with parents, helping them overcome challenges and plans for success.

Moses House Ministries provides help and offers hope to families with young children living in the High Desert. For the past 27 years, we have been a beacon of hope, offering a range of services designed to empower families and individuals to overcome adversity and achieve their God-given potential. We work as a team to see lives changed. We offer helping hands and caring hearts to families in crisis, ensuring children grow up in nurturing homes – free from the risks of abuse and/or neglect. We wrap all of our services around each family, offering a framework for building success, while providing support to grow.

 

As a Case Manager, you will be at the forefront of our ministry, equipping parents and caregivers with the knowledge, strategies, and confidence to navigate various challenges. Your role will involve one-on-one parent support through Case Management sessions designed to identify family strengths and isolate weaknesses. We work to empower families for success by utilizing their unique blend of strengths to overcome challenges and build hope for the future. Case Management is vital in establishing support and structure that guides families into a healthy and thriving future.

Responsibilities include:

  • Provide intake and assessment for new families in an individual appointment and/or group orientation;

  • Assist each participant with preparation of a Family Empowerment Plan, addressing the family’s immediate needs and goals for self-sufficiency or family resiliency, with regular appointments, both center-based and home-based. For monitoring and evaluation;

  • Assist families seeking resources on a one-time emergency basis;

  • Ensure participants receive necessary services from referral agencies and advocate on behalf of clients with referral agencies.

  • Arrange transportation needs with bus vouchers or alternative transportation methods as necessary;

  • Maintain accurate records of each participants visits, needs and other program data;

  • Stay up-to-date on community resources and growing trends in the field;

  • Contribute to a current resource directory;

  • Maintain a positive working relationship with all participants, staff, mentors, and other volunteers, as well as with community partners to whom participants are referred;

  • Assist in coordination of staff support of classes and support groups, available to teach classes where necessary;

 

Qualifications:

  • Bachelor’s degree in Social Work, Psychology or related field preferred;

  • Experience working with individuals in a one-on-one environment developing goals and work plans;

  • Excellent communication and presentation skills;

  • Empathetic and approachable demeanor, with the ability to connect with diverse groups of parents and caregivers;

  • Passion for empowering parents and improving family dynamics;

  • Ability to adapt to participants needs, building support in the midst of family challenges;

  • Bilingual preferred (English/Spanish), but not required;

  • Available to work flexible hours where necessary.
