
Case Management begins with an initial evaluative interview with our case
manager in order to determine and assess individual needs. Following the
initial interview, clients will meet regularly with our
Case Manager to develop, and implement a
plan of action to meet these needs and make positive steps toward their
goals. Our case manager works closely with other community agencies in
order to provide and network clients with the best possible resources.
Each plan of action is individually developed, and
personally tailored for our clients. Whether they are planning on
parenting or adoption, continuing with their education or looking for
work, or dealing with issues surrounding housing, we will work closely
with them, continually monitoring their situations, ensuring they are
receiving the best assistance available.
